A Resource for Professionals
The Optimal Solution for
Discharge Planning
We offer senior care professionals, hospital and rehab social workers, case managers and discharge planners our expertise with an underutilized VA funding source known as Aid & Attendance. The goal is to provide a safe discharge and avoid readmissions.
The VetAssist Program is the Optimal Solution for Many Patients
Long term assistance with activities of daily living (ADLs) isn't covered by Medicare, and sadly, many patients cannot afford care. Since the VA offers benefits for custodial care, geriatric discharge planning should always include questions about Veteran status.
Asking these four simple questions of all patients, both men and women, can change a life.
When a patient says "YES" to all four, offer them to us, and we'll take it from there.

Navigating the VA Maze
The Department of Veterans Affairs is a mammoth bureaucracy that can overwhelm even experienced case managers. We simplify the process for patients needing home care.
Did You Know?
Surviving spouses are often overlooked. If they were married to a wartime Veteran at the time of death, they may qualify for Aid & Attendance benefits nationwide.
2 Paths to Home Care
Veterans Health Admin (VHA)
Provides Homemaker/Home Health Aide services. Dependents and survivors are not eligible for VHA services.
Veterans Benefits Admin (VBA)
Provides "Aid & Attendance" benefit. This is the VetAssist focus.
Monetary Benefits
VA Pension
For wartime veterans. No service-connected disability, combat, or overseas service required.
VA Compensation
For Veterans with service-related disabilities. Per VA’s rating criteria, they consider the Veteran’s level of impairment and determine a rating from 10% to 100%.
